Job Title: Pharmacy Clinical Specialist
Job Summary:
The Pharmacy Clinical Specialist is responsible for providing pharmaceutical care services to patients in a clinical setting. They work closely with healthcare providers to ensure the safe and effective use of medications. This role requires strong clinical knowledge, excellent communication skills, and the ability to work in a fast-paced environment.
Responsibilities and Duties:
- Conduct medication therapy management for patients with chronic diseases
- Collaborate with healthcare providers to optimize medication therapy
- Provide drug information and education to patients and healthcare professionals
- Monitor patients for drug interactions and adverse reactions
- Assist in developing and implementing medication protocols and guidelines
- Participate in medication safety initiatives and quality improvement projects
Qualifications and Skills:
- Doctor of Pharmacy (PharmD) degree
- Active pharmacy license in the state of practice
- Board certification in a clinical specialty (e.g. BCPS)
- Minimum of 2 years of clinical pharmacy experience
- Strong clinical knowledge and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Proficiency in pharmacy software and electronic health records
