Job Title: Home Health Nurse
Job Summary:
We are seeking a compassionate and dedicated Home Health Nurse to provide high-quality medical care to patients in the comfort of their own homes. The ideal candidate will work closely with patients and their families to develop personalized care plans and ensure that all medical needs are met. The Home Health Nurse will also collaborate with other healthcare professionals to coordinate patient care and support a holistic approach to wellness.
Responsibilities and Duties:
- Conduct in-home assessments to evaluate patient's medical needs
- Develop personalized care plans in collaboration with patients and their families
- Provide skilled nursing care, medication management, and wound care as needed
- Monitor and assess patient's condition, progress, and response to treatment
- Educate patients and families on proper care techniques and self-management strategies
- Coordinate care with other healthcare professionals, including doctors, therapists, and social workers
- Maintain accurate and updated patient records and documentation
- Ensure compliance with all healthcare regulations and standards
Qualifications and Skills:
- Valid RN license in the state of practice
- Minimum of 2 years of nursing experience, preferably in home health or long-term care
- Excellent communication and interpersonal skills
- Strong clinical assessment and critical thinking skills
- Ability to work independently and prioritize tasks effectively
- Compassionate and patient-centered approach to care
- Proficient in electronic medical records and healthcare documentation
