We are seeking an experienced and highly motivated Lead Clinical Pharmacist to oversee clinical pharmacy services and ensure the safe, effective, and evidence-based use of medications. In this leadership role, you will manage clinical initiatives, mentor staff, and collaborate with interdisciplinary teams to optimize patient outcomes and support institutional goals.
Lead and manage clinical pharmacy programs across inpatient and/or outpatient settings
Oversee medication therapy management, dosing consultations, and pharmacokinetic monitoring
Collaborate with physicians, nurses, and care teams to develop patient-specific treatment plans
Ensure adherence to clinical protocols, formulary standards, and regulatory requirements
Precept pharmacy residents, students, and staff; support ongoing education and training
Analyze medication use data and contribute to quality improvement and cost-containment initiatives
Participate in Pharmacy & Therapeutics (P&T) Committee meetings and contribute to formulary decisions
Support and implement clinical decision support tools and evidence-based practices
PharmD degree from an accredited college of pharmacy
Current pharmacist license in good standing (state-specific)
Completion of PGY1 residency required; PGY2 or board certification (e.g., BCPS) preferred
5+ years of clinical pharmacy experience, with at least 1–2 years in a leadership or coordinator role
In-depth knowledge of pharmacotherapy, clinical guidelines, and regulatory compliance
Excellent communication, leadership, and decision-making skills
Experience with EMR systems, pharmacy automation, and informatics tools preferred
Lead clinical pharmacy initiatives that improve patient care and safety
