Job Title: Registered Nurse - Home Health
Job Summary:
As a Registered Nurse working in Home Health, you will be responsible for providing quality and compassionate care to patients in their homes. You will assess patients' health needs, develop care plans, administer medications, and educate patients and their families on managing their health conditions.
Responsibilities and Duties:
1. Conduct patient assessments and develop individualized care plans
2. Administer medications and treatments as prescribed by physicians
3. Monitor patients' progress and adjust care plans as needed
4. Educate patients and families on managing health conditions and self-care
5. Coordinate care with other members of the healthcare team, including physicians and therapists
6. Document all patient interactions and care provided accurately and timely
7. Provide emotional support and counseling to patients and their families
Qualifications and Skills:
1. Valid RN license in the state of practice
2. Minimum of 1 year of nursing experience, preferably in home health
3. Strong assessment and critical thinking skills
4. Excellent communication and interpersonal abilities
5. Ability to work independently and as part of a team
6. Strong organizational and time management skills
7. Compassionate and patient-centered approach to care
