Job Title: Registered Nurse - Home Health
Job Summary:
The Registered Nurse - Home Health will provide comprehensive in-home nursing care to patients, working collaboratively with physicians and other healthcare professionals to develop and implement individualized care plans. This role requires excellent clinical skills, strong communication abilities, and a compassionate approach to patient care.
Responsibilities and Duties:
1. Conduct initial assessments and develop care plans for patients receiving home health services.
2. Monitor and evaluate patients' progress, adjusting care plans as needed.
3. Administer medications, perform wound care, and provide other skilled nursing services.
4. Educate patients and their families on disease management and self-care practices.
5. Coordinate with other healthcare providers to ensure continuity of care.
6. Document all patient interactions and care provided in a timely and accurate manner.
7. Participate in interdisciplinary team meetings to discuss patient progress and collaborate on care plans.
8. Adhere to all legal and ethical standards of nursing practice.
9. Provide emotional support and encouragement to patients and their families.
Qualifications and Skills:
1. Current RN license in the state of practice.
2. Minimum of 1 year of experience in home health nursing preferred.
3. Strong clinical assessment and critical thinking skills.
4. Excellent communication and interpersonal abilities.
5. Ability to work independently and manage time effectively.
6. Proficiency in electronic medical record documentation.
7. BLS certification required; ACLS certification preferred.
8. Reliable transportation and valid driver's license.
9. Ability to lift and move patients as needed.
