Job Title: Registered Nurse - Long Term Care (LTC)
Job Summary:
We are seeking a compassionate and experienced Registered Nurse to join our Long Term Care facility. The ideal candidate will provide quality care to residents requiring long-term care services. The Registered Nurse will work closely with other healthcare professionals to assess, plan, implement, and evaluate resident care in a supportive and collaborative environment.
Responsibilities and Duties:
1. Assess and monitor residents' health status and collaborate with the healthcare team to develop individualized care plans.
2. Administer medications and treatments in accordance with physician orders and facility policies.
3. Provide direct care to residents, including wound care, IV therapy, and managing chronic conditions.
4. Communicate effectively with residents, families, and healthcare team members to ensure continuity of care.
5. Document resident care accurately and timely in accordance with regulatory standards.
6. Supervise and mentor nursing staff, including Certified Nursing Assistants and Licensed Practical Nurses.
7. Participate in quality improvement initiatives and infection control measures.
8. Maintain a safe and therapeutic environment for residents and staff.
9. Adhere to ethical and professional standards of nursing practice.
Qualifications and Skills:
- Active Registered Nurse license in the state of employment
- Minimum of 2 years of nursing experience in long-term care or a related setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Basic computer skills for documentation and communication
- BLS certification (ACLS and PALS may be required depending on facility)
