Job Summary:
We are seeking a compassionate and dedicated Registered Nurse to join our team as a Home Health RN. As a Home Health RN, you will provide care to patients in their homes with various medical needs and manage their overall care plan. The ideal candidate will have experience in clinical coordination, discharge planning, and patient education.
Responsibilities and Duties:
• Assess and manage patient care needs in their homes
• Develop and implement care plans and evaluate the effectiveness of ongoing care
• Provide education to patients and their families on their medical conditions, treatment plan, and medication management
• Clinical coordination and discharge planning for patients
• Communicate and collaborate with other healthcare professionals to ensure continuity of care
• Ensure adherence to all relevant regulations, policies, and procedures
Qualifications and Skills:
• Active RN license in the state of Vermont
• Minimum of 2 years of nursing experience, with at least 1 year in home health or a related field
• Excellent clinical assessment and critical thinking skills
• Strong communication, interpersonal, and organizational skills
• Ability to work independently and with a team
• Proficient in the use of electronic medical records (EMR)
