Job title: Hospice Registered Nurse
Job Summary:
We are seeking a compassionate and dedicated Hospice Registered Nurse to provide end-of-life care and support to patients and their families. The Hospice RN will work closely with a multidisciplinary team to coordinate care, manage symptoms, and ensure the comfort and dignity of patients in their final days.
Responsibilities and Duties:
- Conduct thorough assessments of patients' physical, emotional, and psychosocial needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and manage symptoms, provide pain management, and promote comfort measures
- Educate patients and families on end-of-life care, advance directives, and available resources
- Provide emotional support and guidance to patients and families throughout the hospice journey
- Maintain accurate and timely documentation of all care provided
- Collaborate with physicians, social workers, chaplains, and other healthcare professionals to ensure holistic care
Qualifications and Skills:
- Current RN license in the state of practice
- Minimum of 2 years of nursing experience, preferably in hospice or palliative care
- Excellent communication and interpersonal skills
- Strong critical thinking and problem-solving abilities
- Compassionate and empathetic attitude towards patients and families
- Ability to work effectively in a fast-paced and emotionally demanding environment
- Knowledge of end-of-life care principles and practices
- BLS certification
