Job Title: Home Health Registered Nurse (RN)
Job Summary:
We are seeking a compassionate and dedicated Home Health RN to provide high-quality nursing care to patients in the comfort of their own homes. The Home Health RN will work closely with patients and their families to develop personalized care plans and ensure the highest level of patient satisfaction.
Responsibilities and Duties:
- Conduct initial assessments of patients' health status
- Develop and implement individualized care plans
- Provide skilled nursing care, including wound care, IV therapy, and medication management
- Monitor and evaluate patients' progress
- Educate patients and their families on health management and disease prevention
- Collaborate with other healthcare professionals to provide comprehensive care
- Maintain accurate and up-to-date patient documentation
- Adhere to all federal, state, and local regulations and guidelines
Qualifications and Skills:
- Valid RN license in the state of practice
- Minimum of 1 year of clinical nursing experience
- BLS certification
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively
- Experience in home health or hospice care is preferred
- Compassionate and patient-centered approach to care
