Job Title: Hospital Social Worker
Job Summary:
We are seeking a compassionate and dedicated Hospital Social Worker to provide support and assistance to patients and their families during their hospital stay. The Hospital Social Worker will work closely with healthcare teams to assess patients' needs, develop treatment plans, and connect them with resources and services within the community.
Responsibilities and Duties:
- Conduct psychosocial assessments of patients to identify their needs and concerns
- Collaborate with healthcare teams to develop and implement treatment plans
- Provide counseling and emotional support to patients and their families
- Assist patients in accessing financial and social resources such as housing, transportation, and healthcare
- Advocate for patients' rights and ensure they receive appropriate care and services
- Maintain accurate and confidential patient records
Qualifications and Skills:
- Master's degree in Social Work (MSW) from an accredited program
- Licensed Clinical Social Worker (LCSW) preferred
- Experience working in a healthcare setting, preferably in a hospital
- Strong communication and interpersonal skills
- Ability to work collaboratively with interdisciplinary teams
- Knowledge of community resources and social service programs
- Compassionate and empathetic approach to patient care
