Job title: Inpatient Hospital Pharmacist
Job Summary:
The Inpatient Hospital Pharmacist is responsible for providing pharmaceutical care to patients admitted to the hospital. This includes reviewing medication orders, preparing and dispensing medications, providing drug information to healthcare providers, and monitoring patients for drug interactions and adverse reactions.
Responsibilities and Duties:
1. Review medication orders from healthcare providers and ensure accuracy and appropriateness.
2. Prepare and dispense medications according to established protocols.
3. Provide drug information to healthcare providers and patients.
4. Monitor patients for drug interactions and adverse reactions.
5. Collaborate with other healthcare providers to ensure safe and effective medication use.
6. Maintain accurate medication records and documentation.
7. Participate in interdisciplinary rounds and committees to improve patient care.
8. Stay current on new drugs, therapies, and guidelines in hospital pharmacy practice.
9. Assist in training and mentoring pharmacy students and residents.
10. Follow all hospital policies and procedures related to medication management.
Qualifications and Skills:
1. Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy.
2. Licensed as a pharmacist in the state of practice.
3. Completion of a pharmacy residency program or equivalent experience in hospital pharmacy practice.
4. Strong clinical knowledge and problem-solving skills.
5. Excellent communication and interpersonal skills.
6. Ability to work independently and collaboratively in a fast-paced environment.
7. Attention to detail and accuracy in medication preparation and dispensing.
8. Ability to prioritize tasks and manage time effectively.
9. Proficiency in using pharmacy computer systems and software.
10. Commitment to continuous professional development and lifelong learning.
