Job Title: Hospice/Bereavement Lead Coordinator
Job Summary:
The Hospice/Bereavement Lead Coordinator is responsible for overseeing and managing bereavement services within a hospice care organization. They provide support, guidance, and resources to individuals and families who are coping with grief and loss.
Responsibilities and Duties:
1. Develop and implement bereavement support programs for hospice patients and their families.
2. Coordinate and facilitate support groups for individuals grieving the loss of a loved one.
3. Provide individual counseling and support services to bereaved individuals as needed.
4. Collaborate with interdisciplinary team members to ensure holistic care for hospice patients and families.
5. Maintain accurate and confidential records of all bereavement services provided.
6. Participate in community outreach and education activities to raise awareness about grief and bereavement support resources.
7. Coordinate memorial services and other special events to honor and remember hospice patients.
8. Stay current on best practices and research in the field of bereavement support.
Qualifications and Skills:
- Bachelor's degree in social work, counseling, psychology, or a related field (Master's degree preferred)
- Certification in grief counseling or bereavement support
- Minimum of 3 years of experience in a hospice or palliative care setting
- Strong communication and interpersonal skills
- Compassionate and empathetic demeanor
- Ability to work effectively in a team environment
- Knowledge of ethical and legal standards related to grief counseling
