Job Title: Social Worker (Home Hospice)
Job Summary:
The Social Worker (Home Hospice) provides emotional support, counseling, and resources to patients and their families facing terminal illness. They work with interdisciplinary teams to ensure the holistic care of patients and help them navigate end-of-life decisions.
Responsibilities and Duties:
1. Conduct psychosocial assessments to identify patient and family needs.
2. Develop care plans that address emotional, spiritual, and practical needs.
3. Provide counseling and support to patients and families.
4. Coordinate resources such as community services, financial assistance, and legal support.
5. Advocate for patients' rights and preferences.
6. Collaborate with healthcare professionals to ensure comprehensive care.
7. Document patient progress and maintain accurate records.
8. Participate in interdisciplinary team meetings.
9. Educate patients and families on hospice care and end-of-life issues.
10. Provide bereavement support to families after the loss of a loved one.
Qualifications and Skills:
1. Master's degree in Social Work from an accredited institution.
2. Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) preferred.
3. Minimum of 2 years of experience in hospice or palliative care.
4. Strong communication and interpersonal skills.
5. Ability to work independently and as part of a team.
6. Compassionate and empathetic nature.
7. Knowledge of end-of-life care, grief, and bereavement issues.
8. Proficiency in medical documentation and record-keeping.
