Job Title: Long Term Care (LTC) Geriatric LPN
Job Summary:
We are seeking a qualified Licensed Practical Nurse (LPN) to join our team at a Long Term Care (LTC) facility specializing in geriatric care. The LPN will be responsible for providing direct patient care, administering medications, and implementing individualized care plans for elderly residents.
Responsibilities and Duties:
1. Provide nursing care in accordance with facility policies and procedures
2. Administer medications and treatments as prescribed by physicians
3. Monitor residents' health status and report any changes to the nursing team
4. Assist with activities of daily living, including bathing, dressing, and feeding
5. Collaborate with the interdisciplinary team to develop and implement care plans
6. Educate residents and families on proper care techniques and health maintenance
7. Maintain accurate and up-to-date medical records
8. Follow infection control guidelines to ensure a safe and clean environment for residents
Qualifications and Skills:
1. Current LPN license in the state of employment
2. Minimum of 1 year of experience in long term care or geriatric nursing
3. Strong communication and interpersonal skills
4. Ability to work effectively in a fast-paced environment
5. Compassionate and patient-centered approach to care
6. Knowledge of geriatric health issues and age-related diseases
7. CPR certification
8. Ability to prioritize tasks and manage time effectively
