Job Title: Administrative Assistant (Care Coordination/Case Management)
Job Summary:
We are seeking a highly organized and detail-oriented individual to join our team as an Administrative Assistant for our Care Coordination/Case Management department. The ideal candidate will be responsible for providing administrative support to the care coordination team, assisting with client intake and scheduling, maintaining client records, and ensuring efficient communication and coordination between team members.
Responsibilities and Duties:
1. Assist with client intake and scheduling appointments
2. Maintain accurate and up-to-date client records
3. Coordinate communication between clients, caregivers, and healthcare providers
4. Assist with data entry and documentation tasks
5. Assist with coordinating care plans and ensuring all necessary services are in place
6. Provide administrative support to the care coordination team as needed
Qualifications and Skills:
1. High school diploma or equivalent required
2. Prior experience in a healthcare or human services setting preferred
3. Strong organizational and multitasking skills
4. Excellent communication and interpersonal skills
5. Proficiency in Microsoft Office Suite
6. Ability to work independently and as part of a team
7. Attention to detail and ability to prioritize tasks
8. Knowledge of HIPAA regulations and confidentiality requirements
