Job Title: Long Term Care (LTC) Nurse
Job Summary:
The Long Term Care (LTC) Nurse is responsible for providing compassionate and quality care to residents in long-term care facilities. They work closely with other healthcare professionals to create and implement care plans tailored to the individual needs of each resident. The LTC Nurse also monitors and evaluates residents' health status, administers medications, and provides emotional support to residents and their families.
Responsibilities and Duties:
1. Assess residents' health status and create individualized care plans
2. Administer medications and treatments as prescribed by healthcare providers
3. Monitor and evaluate residents' progress and make necessary adjustments to care plans
4. Collaborate with interdisciplinary teams to ensure holistic care for residents
5. Provide emotional support to residents and their families
6. Educate residents and their families on proper healthcare practices
7. Maintain accurate and up-to-date medical records
8. Comply with all healthcare regulations and protocols
Qualifications and Skills:
1. Valid nursing license in the state of practice
2. Prior experience in long term care or geriatric nursing preferred
3. Strong communication and interpersonal skills
4. Ability to work effectively in a fast-paced environment
5. Compassionate and patient-centered approach to care
6. Knowledge of healthcare regulations and protocols
7. Ability to collaborate effectively with interdisciplinary teams
