Job Title: Inpatient Pharmacist
Job Summary:
The Inpatient Pharmacist is responsible for providing pharmaceutical care to patients admitted to the hospital. They work closely with physicians and other healthcare professionals to ensure the safe and effective use of medications.
Responsibilities and Duties:
1. Review patient medication orders and ensure accuracy and appropriateness.
2. Prepare and dispense medications according to physician orders.
3. Monitor medication therapy for efficacy and potential adverse effects.
4. Provide drug information to healthcare professionals and patients.
5. Collaborate with the healthcare team to develop individualized treatment plans.
6. Participate in medication reconciliation and transition of care processes.
7. Evaluate laboratory and clinical data to optimize medication therapy.
8. Train and supervise pharmacy technicians and support staff.
9. Maintain accurate and complete medication records.
10. Comply with state and federal regulations regarding pharmacy practice.
Qualifications and Skills:
1. Doctor of Pharmacy (Pharm.D.) degree.
2. Licensed pharmacist in the state of practice.
3. Residency training in hospital pharmacy preferred.
4. Strong communication and collaboration skills.
5. Ability to work in a fast-paced and high-pressure environment.
6. Knowledge of pharmacokinetics, pharmacodynamics, and drug interactions.
7. Attention to detail and accuracy.
8. Ability to prioritize and multitask effectively.
9. Strong problem-solving skills.
10. Proficiency in pharmacy computer systems and software.
