Job Title: Registered Nurse Case Manager
Job Summary:
As a Registered Nurse Case Manager, you will be responsible for coordinating and providing care for patients in a variety of settings. You will work closely with other healthcare professionals to ensure that patients receive the best possible care and make sure that their needs are met throughout their treatment.
Responsibilities and Duties:
- Assessing patient needs and developing individualized care plans
- Coordinating care across multiple providers and settings
- Advocating for patients and their families
- Educating patients about their conditions and treatment options
- Monitoring patient progress and adjusting care plans as needed
- Ensuring that patients receive the necessary resources and support to achieve optimal health outcomes
- Collaborating with other healthcare professionals to coordinate care
Qualifications and Skills:
- Active RN license
- Bachelors degree in nursing (BSN) preferred
- Previous experience in case management or care coordination
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of healthcare systems and resources
- Ability to multitask and prioritize tasks
- Proficiency in medical documentation and record-keeping
