Job Title: Travel Registered Nurse (RN) Case Manager Hospice
Job Summary:
We are seeking a compassionate and experienced Travel Registered Nurse (RN) Case Manager for our Hospice program. The ideal candidate will be responsible for coordinating and providing quality patient care to individuals in end-of-life situations. This position requires excellent communication skills, a strong understanding of hospice care principles, and the ability to work collaboratively with a multidisciplinary team.
Responsibilities and Duties:
- Conduct initial assessments of patients and develop individualized care plans
- Coordinate and oversee the delivery of hospice care services to ensure quality and efficiency
- Work closely with physicians, social workers, and other healthcare professionals to provide holistic care to patients
- Monitor patient progress and update care plans as needed
- Provide education and support to patients and their families regarding end-of-life care options
- Ensure compliance with all regulatory requirements and standards of practice
Qualifications and Skills:
- Active RN license in the state of practice
- Minimum of 2 years of experience in hospice care
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work autonomously and as part of a team
- Willingness to travel to various locations as needed
