Job Title: LPN, Long Term Care (LTC)
Job Summary:
As an LPN in Long Term Care (LTC), you will be responsible for providing compassionate and quality nursing care to residents in long-term care facilities. You will work closely with other healthcare professionals to ensure the physical and emotional well-being of residents.
Responsibilities and Duties:
1. Administer medications and treatments as prescribed by physicians
2. Monitor and assess patients' health status
3. Create and update care plans for residents
4. Collaborate with healthcare team members to provide comprehensive care
5. Assist residents with activities of daily living
6. Educate residents and their families on healthcare practices
7. Maintain accurate medical records
8. Adhere to all legal and ethical standards of nursing practice
Qualifications and Skills:
1. Graduation from an accredited LPN program
2. Current LPN license in the state of employment
3. Experience in long-term care preferred
4. Excellent communication and interpersonal skills
5. Ability to work well in a team environment
6. Strong attention to detail
7. Compassion and empathy towards residents
8. Ability to handle stressful situations with professionalism
