Job Summary:
We are seeking a compassionate and dedicated Sub Acute Care Center Nurse to join our skilled nursing facility. The Sub Acute Care Center Nurse will be responsible for providing high-quality care to patients with complex medical conditions in a sub-acute care setting. The ideal candidate will have a strong background in nursing, excellent communication skills, and a commitment to delivering holistic care to patients.
Responsibilities and Duties:
- Assessing and monitoring patients' medical conditions
- Administering medications and treatments as prescribed by physicians
- Collaborating with the interdisciplinary team to create individualized care plans
- Providing emotional support and education to patients and their families
- Maintaining accurate and up-to-date patient records
- Implementing infection control measures to ensure the safety of patients and staff
- Participating in quality improvement initiatives to enhance patient care outcomes
Qualifications and Skills:
- Valid RN license in the state of practice
- Minimum of 2 years of experience in a sub-acute care setting
- BLS certification
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work effectively as part of a team
- Knowledge of electronic medical records systems
