Job title: Skilled Nursing (SNF) Registered Nurse - Wound Care
Job Summary:
We are seeking a skilled and compassionate Registered Nurse with expertise in wound care to join our skilled nursing facility (SNF) team. The ideal candidate will be responsible for assessing, planning, implementing, and evaluating wound care treatments for residents with acute or chronic wounds.
Responsibilities and Duties:
- Conduct comprehensive assessments of resident wounds and develop individualized care plans
- Administer treatments and medications as prescribed by healthcare providers
- Educate patients, families, and staff on wound care best practices
- Collaborate with interdisciplinary team members to ensure optimal wound healing outcomes
- Monitor wound progress and make adjustments to care plans as needed
- Maintain accurate and thorough documentation of all wound care interventions
- Adhere to infection control protocols and safety standards in wound care practices
Qualifications and Skills:
- Active Registered Nurse (RN) license in the state of practice
- Minimum of 2 years of experience in a skilled nursing facility or acute care setting
- Certification in wound care management (WCC) preferred
- Strong clinical assessment and critical thinking skills
- Excellent interpersonal and communication abilities
- Ability to work collaboratively in a team environment
- Proficiency in electronic medical records (EMR) systems
