Job Summary:
The Registered Nurse in Long Term Care (LTC) and Long Term Acute Care settings provides comprehensive nursing care to patients with chronic illnesses or complex medical conditions over an extended period of time. They collaborate with healthcare teams to develop and implement care plans that promote optimal health and well-being for patients in these specialized care settings.
Responsibilities and Duties:
- Assessing and monitoring patients' medical conditions, changes in health status, and responses to treatments
- Administering medications and treatments as prescribed by healthcare providers
- Collaborating with interdisciplinary teams to develop individualized care plans for patients
- Providing emotional support and education to patients and their families
- Documenting patient care activities and communicating effectively with healthcare team members
- Ensuring compliance with regulatory standards and best practices in nursing care
Qualifications and Skills:
- Current RN licensure in the state of practice
- Minimum of 1-2 years of nursing experience in Long Term Care or Long Term Acute Care settings
- Strong clinical assessment skills and ability to make sound nursing judgments
- Excellent communication and interpersonal skills
- Ability to collaborate effectively with healthcare team members
