Job Title: Hospice RN, Home Health RN
Job Summary:
We are seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Hospice RN / Home Health RN. In this role, you will be responsible for providing quality end-of-life care and medical support to patients in their homes. You will work closely with patients, families, and interdisciplinary healthcare teams to ensure the highest level of comfort and dignity for patients in their final days.
Responsibilities and Duties:
1. Provide skilled nursing care to patients in their homes, including medication management, wound care, and symptom management.
2. Develop and implement individualized care plans in collaboration with the interdisciplinary team.
3. Educate patients and their families on end-of-life care and assist with advance care planning.
4. Coordinate care with other healthcare professionals, such as physicians, social workers, and therapists.
5. Provide emotional support and counseling to patients and families throughout the hospice process.
6. Maintain accurate and up-to-date patient records and documentation.
7. Monitor and evaluate patient progress and adjust care plans as needed.
8. Participate in quality improvement initiatives and ongoing education to ensure best practices in hospice care.
Qualifications and Skills:
1. Active RN license in the state of practice.
2. Minimum of 2 years of experience in hospice or home health care.
3. BLS certification.
4. Strong communication and interpersonal skills.
5. Compassionate and empathetic attitude towards patients and families.
6. Ability to work independently and as part of a multidisciplinary team.
7. Excellent organizational and time management skills.
8. Knowledge of Medicare and Medicaid regulations related to hospice care.
