Job Title: Hospice RN, Home Health RN
Job Summary:
The Hospice RN, Home Health RN provides compassionate and high-quality care to patients in the comfort of their own homes. They work closely with patients, families, and interdisciplinary teams to provide comprehensive and personalized end-of-life care.
Responsibilities and Duties:
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient progress and make adjustments to care plans as needed
- Provide emotional support and education to patients and their families
- Coordinate care with other healthcare providers and community resources
- Ensure compliance with all regulatory requirements and standards of practice
Qualifications and Skills:
- Current RN license in the state of practice
- Minimum of 1 year of experience in hospice or home health care
- BLS certification
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and empathetic demeanor
