Job Title: Skilled Nursing (SNF) / Long Term Care (LTC) Nurse
Job Summary:
We are seeking a compassionate and skilled nurse to join our Skilled Nursing (SNF) / Long Term Care (LTC) facility. The ideal candidate will be responsible for providing high-quality care to our residents and ensuring their physical and emotional well-being.
Responsibilities and Duties:
1. Administer medications and treatment plans as prescribed by physicians
2. Monitor and assess residents' health status on a regular basis
3. Collaborate with the interdisciplinary team to develop individual care plans
4. Assist with activities of daily living, including bathing, dressing, and eating
5. Educate residents and their families on health management and disease prevention
6. Maintain accurate and detailed medical records
7. Respond to medical emergencies and provide immediate care
8. Adhere to all facility policies and procedures
9. Participate in staff meetings and continuing education programs
Qualifications and Skills:
1. Valid RN or LPN license
2. Minimum of 2 years of experience in a Skilled Nursing (SNF) or Long Term Care (LTC) setting
3. Strong clinical skills and knowledge of geriatric care
4. Excellent communication and interpersonal skills
5. Ability to work both independently and as part of a team
6. Basic computer skills
7. CPR certification
8. Compassionate and patient-focused approach to care
