Job Title: Hospice RN, Home Health RN
Job Summary:
We are seeking a compassionate and dedicated Registered Nurse to join our team as a Hospice and Home Health RN. In this role, you will provide skilled nursing care to patients in their homes, ensuring their comfort and dignity during their end-of-life journey. You will work closely with patients, families, and multidisciplinary healthcare teams to provide high-quality care and support.
Responsibilities and Duties:
- Perform comprehensive assessments of patients’ physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team
- Provide skilled nursing care, including wound care, medication management, and symptom management
- Educate and support patients and families on end-of-life care and advance care planning
- Coordinate care with other healthcare providers, including physicians, therapists, and social workers
- Provide emotional support to patients and families during difficult times
- Document patient care and progress accurately and in a timely manner
- Adhere to all regulatory and compliance requirements
Qualifications and Skills:
- Valid Registered Nurse (RN) license
- Minimum of 2 years of nursing experience, preferably in hospice or home health care
- BLS certification
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and empathetic attitude
- Knowledge of end-of-life care principles and practices
- Proficiency in electronic medical records (EMR) systems
