Job Title: Long Term Care (LTC) Registered Nurse - Acute Care
Job Summary:
As a Long Term Care Registered Nurse specializing in Acute Care, you will be responsible for providing high-quality nursing care to patients in long-term care facilities who require acute medical attention. Your primary focus will be on managing and treating acute medical conditions, overseeing medication administration, and collaborating with physicians and healthcare professionals to ensure the best possible outcomes for your patients.
Responsibilities and Duties:
1. Assess and monitor patients' acute medical conditions, symptoms, and vital signs.
2. Administer medications and treatments as prescribed by physicians.
3. Collaborate with interdisciplinary team members to develop individualized care plans for patients.
4. Educate patients and their families on treatment plans, medications, and self-care practices.
5. Monitor changes in patients' conditions and intervene promptly to prevent complications.
6. Document patient care activities and communicate effectively with healthcare team members.
7. Provide emotional support and comfort to patients and their families during challenging times.
8. Maintain a clean and safe environment for patients and adhere to infection control protocols.
9. Participate in quality improvement initiatives and continuing education to enhance nursing skills.
Qualifications and Skills:
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of 2 years of experience in acute care nursing, preferably in a long-term care setting.
- BLS and ACLS certifications (may vary by facility).
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively in a fast-paced and demanding environment.
