Job Title: Long Term Care (LTC) Nurse
Job Summary:
As a Long Term Care Nurse, you will be responsible for providing quality nursing care to residents in long-term care facilities. You will assess patient needs, create care plans, administer medications, and collaborate with other healthcare professionals to ensure the overall well-being of residents.
Responsibilities and Duties:
1. Conduct comprehensive assessments of residents' health status
2. Develop and implement individualized care plans
3. Administer medications and treatments as prescribed by healthcare providers
4. Monitor and report changes in residents' health status
5. Collaborate with physicians, therapists, and other healthcare professionals
6. Educate residents and their families about healthcare needs and treatment options
Qualifications and Skills:
1. Active RN or LPN license in the state of practice
2. Previous experience in long-term care or geriatric nursing preferred
3. Strong clinical assessment and critical thinking skills
4. Excellent communication and interpersonal skills
5. Ability to work effectively in a team environment
6. Compassionate and patient-centered approach to care
