Job Title: Staff Pharmacist (Ambulatory Clinic)
Job Summary:
We are seeking a dedicated and experienced Staff Pharmacist to join our ambulatory clinic team. The Staff Pharmacist will be responsible for providing pharmaceutical care in an outpatient setting, ensuring safe and effective medication use for patients. This role involves collaborating with healthcare providers, conducting medication therapy management, educating patients on proper medication usage, and maintaining accurate patient records.
Responsibilities and Duties:
- Dispensing medications accurately and safely to ambulatory clinic patients
- Reviewing medication orders for accuracy and appropriateness
- Providing drug information to healthcare providers and patients
- Monitoring patient medication therapy and outcomes
- Collaborating with interdisciplinary healthcare team members
- Participating in quality improvement initiatives
- Ensuring compliance with state and federal regulations
- Maintaining a clean and organized pharmacy workspace
Qualifications and Skills:
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license in the state of practice
- Prior experience in ambulatory care or clinic pharmacy preferred
- Strong clinical knowledge and critical thinking skills
- Excellent communication and interpersonal abilities
- Attention to detail and accuracy in medication dispensing
- Ability to work independently and as part of a team
- Proficiency in pharmacy software and electronic health records
