The Home Health RN – Clinic provides skilled nursing care to patients in their homes and coordinates with the clinic team to ensure continuity of care. This role involves conducting clinical assessments, managing chronic conditions, delivering treatments, and educating patients and families to support independent living and reduce hospital readmissions.
Perform in-home nursing assessments and deliver personalized patient care based on the plan of care.
Collaborate with clinic providers, case managers, and therapists to coordinate care and follow-ups.
Administer medications, wound care, IV therapy, and other skilled nursing interventions.
Educate patients and families about disease management, medication compliance, and safety.
Document clinical findings accurately in the electronic health record (EHR).
Monitor patient progress and communicate changes in condition to the healthcare team.
Ensure compliance with clinical protocols, regulatory standards, and infection control.
Participate in team meetings and contribute to quality improvement efforts.
Licensure: Active RN license in [State]
Education: Associate or Bachelor’s degree in Nursing (BSN preferred)
Certifications: BLS required; additional certifications (e.g., OASIS, wound care) preferred
Experience: 1–2 years of experience in home health, clinic, or community nursing preferred
Strong critical thinking, communication, and time-management skills
Reliable transportation and valid driver’s license required for home visits
