The LPN – Primary Care Clinic provides patient-centered nursing care in an outpatient clinic setting. The LPN works collaboratively with physicians, nurse practitioners, and other care team members to support routine check-ups, chronic disease management, preventive care, and minor procedures, ensuring smooth clinical operations and exceptional patient service.
Assist with patient intake, including vitals, history, and screening assessments.
Administer immunizations, injections, and prescribed medications under physician orders.
Support chronic care management (e.g., diabetes, hypertension, asthma) through patient education and follow-up.
Prepare patients and assist providers with exams, procedures, and diagnostic tests.
Document all patient interactions accurately in the electronic health record (EHR).
Ensure clinic rooms and supplies are clean, stocked, and organized.
Provide patient education on health promotion, medications, and treatment plans.
Communicate effectively with patients, families, and interdisciplinary care teams.
Licensure: Active and unencumbered LPN license in [State].
Education: Graduate of an accredited LPN program.
Certifications: BLS required.
Experience: 1–2 years of LPN experience in a clinic or outpatient setting preferred.
Excellent interpersonal, communication, and organizational skills.
Experience with EMR systems (e.g., Epic, Cerner) preferred.
