Licensed Practical Nurse (LPN) – Long Term Care (LTC)

📌 Job Summary:

We are seeking a compassionate and dedicated LPN for Long Term Care (LTC) to provide high-quality nursing care to residents in a long-term care facility. The LPN will work under the supervision of Registered Nurses (RNs) and physicians to deliver patient-centered care, ensuring comfort, dignity, and well-being.

✅ Key Responsibilities:

Provide direct nursing care to residents, including administering medications and treatments.
Monitor and document resident health status, vital signs, and progress.
Assist residents with daily living activities while promoting independence.
Collaborate with RNs, physicians, and interdisciplinary teams for care planning.
Maintain accurate medical records and ensure compliance with care standards.
Respond to emergencies and provide appropriate interventions.
Support residents’ families with education and emotional care.
Adhere to infection control, safety, and healthcare regulations.

🎓 Qualifications:

Required:

Current, valid LPN/LVN license.
CPR/BLS certification.
Strong communication and organizational skills.
Compassionate and patient-focused approach to care.

Preferred:

Previous experience in Long Term Care, Nursing Home, or Skilled Nursing Facility (SNF).
Knowledge of EMR/EHR systems.
