📝 Job Description:

We are seeking a dedicated Home Health Registered Nurse (RN) to provide compassionate and skilled nursing care to patients in their homes. The ideal candidate will coordinate with physicians, families, and other healthcare professionals to ensure the highest quality of care and promote optimal health outcomes.

⚕️ Key Responsibilities:

• Provide in-home nursing care, including assessments, wound care, medication administration, and IV therapy.

• Develop, implement, and evaluate patient care plans.

• Monitor patient progress and communicate with physicians and family members.

• Educate patients and caregivers on disease prevention, treatment, and recovery.

• Maintain accurate clinical documentation and comply with agency policies.

• Ensure adherence to infection control, safety, and healthcare standards.

🎓 Qualifications:

• Active Registered Nurse (RN) license (state-specific).

• Minimum 2+ years of clinical nursing experience (Home Health preferred).

• Strong assessment and critical thinking skills.

• Excellent communication, time management, and organizational skills.

• Valid driver’s license and reliable transportation.

💼 Preferred Experience:

• Experience with OASIS documentation.

• Knowledge of home health regulations and Medicare compliance.

• Familiarity with electronic health record (EHR) systems.
