Care Coordinator Specialist (Clerical / Office Assistant / Support)
The Care Coordinator Specialist plays a key administrative and support role within the healthcare team, ensuring that patients receive efficient, timely, and coordinated care. This position involves clerical duties, communication with patients and providers, and accurate documentation to support quality patient outcomes.
Coordinate and schedule patient appointments, referrals, and follow-ups.
Serve as a liaison between patients, healthcare providers, insurance companies, and support staff.
Maintain and update electronic medical records (EMR) accurately.
Perform clerical tasks such as data entry, scanning, filing, and correspondence management.
Verify insurance eligibility, authorizations, and billing information.
Assist patients in navigating care plans and provide education about services.
Ensure all documentation meets compliance, confidentiality, and HIPAA standards.
Track patient progress and assist in maintaining continuity of care.
Support the clinical team with reports, communications, and administrative projects.
High school diploma or equivalent (Associate’s or Bachelor’s degree preferred).
Prior experience in healthcare administration, clerical, or care coordination preferred.
Strong organizational and multitasking skills.
Excellent communication and customer service abilities.
Proficiency with Microsoft Office and Electronic Medical Records (EMR) systems.
Knowledge of medical terminology and insurance processes is an advantage.
