Clinical Social Worker – Case Management

Job Summary:

We are seeking a compassionate and skilled Clinical Social Worker to join our Case Management team. The Clinical Social Worker provides emotional support, counseling, and resource coordination to patients and families, ensuring they receive comprehensive care throughout their treatment journey. This role combines clinical expertise with care coordination to promote optimal health outcomes and smooth transitions of care.

Key Responsibilities:

• Assess patients’ psychosocial, emotional, and environmental needs to support treatment plans.

• Provide crisis intervention, counseling, and therapeutic support for patients and families.

• Develop and implement individualized care plans in collaboration with healthcare teams.

• Coordinate discharge planning and community resource referrals.

• Serve as a liaison between patients, healthcare providers, and external agencies.

• Advocate for patients’ rights and access to appropriate resources.

• Document all assessments, plans, and interventions accurately in compliance with regulations.

• Support case management functions to ensure continuity of care across all services.

• Participate in interdisciplinary meetings and quality improvement initiatives.

Qualifications:

• Master’s Degree in Social Work (MSW) required.

• Current state licensure (LCSW or equivalent).

• Prior experience in healthcare, case management, or behavioral health preferred.

• Strong understanding of community resources, psychosocial assessment, and discharge planning.

• Excellent communication, problem-solving, and documentation skills.

• Empathetic, professional, and patient-centered approach.

Work Environment:

• Hospital, clinic, or long-term care setting.

• Collaborative, multidisciplinary care environment.

• Full-time or part-time schedule depending on facility needs.
