We are seeking a compassionate and dedicated LVN/LPN to join our Long Term Care (LTC) and Geriatric Nursing team. The ideal candidate will provide high-quality nursing care to elderly residents in a long-term care or skilled nursing facility. This role involves assisting residents with daily activities, administering medications, and collaborating with the interdisciplinary care team to promote comfort, dignity, and optimal health outcomes.
Provide direct patient care under the supervision of an RN or physician.
Administer prescribed medications and treatments in accordance with facility policies.
Monitor and record patient vital signs, symptoms, and responses to treatment.
Assist residents with activities of daily living (ADLs), including hygiene, mobility, and nutrition.
Maintain accurate and up-to-date medical records and documentation.
Communicate effectively with residents, families, and healthcare team members.
Participate in care planning, discharge planning, and patient education.
Uphold infection control standards and safety protocols.
Provide emotional support and promote a positive, respectful environment for all residents.
Valid LVN (Licensed Vocational Nurse) or LPN (Licensed Practical Nurse) license in the state of practice.
Minimum 1–2 years of experience in long-term care, geriatric, or skilled nursing settings preferred.
Current BLS certification required.
Strong communication, observation, and critical thinking skills.
Compassionate, patient-centered approach to care.

Collaborative, multidisciplinary healthcare team.
May include weekends, holidays, and rotating shifts.
