The Care Manager – Med Surg Adult is responsible for coordinating and managing the care of adult medical-surgical patients to ensure quality outcomes and efficient resource utilization. This role focuses on patient advocacy, discharge planning, and interdisciplinary collaboration to support a smooth transition of care throughout the hospital stay.
Assess and evaluate patients’ medical, social, and emotional needs upon admission.
Develop and implement individualized care plans in collaboration with the healthcare team.
Coordinate discharge planning, referrals, and post-acute services.
Monitor patient progress and adjust care plans to optimize outcomes.
Facilitate communication among physicians, nurses, patients, and families.
Identify barriers to care and work proactively to remove them.
Ensure compliance with hospital policies, regulatory standards, and payer requirements.
Educate patients and families about available healthcare resources and follow-up care.
Registered Nurse (RN) license required.
BSN preferred; Case Management Certification (CCM or ACM) is an advantage.
Minimum 2–3 years of clinical experience in Med-Surg or acute care settings.
Strong assessment, communication, and organizational skills.
Knowledge of utilization management and discharge planning processes.
Hospital-based inpatient Med-Surg unit.
Interdisciplinary collaboration with physicians, social workers, and other care team members.
Involves patient/family interaction, documentation, and coordination with insurance providers.
