Patient Service Coordinator / Outpatient Coordinator (REMOTE) – Community Health Worker

🩺 Job Summary:

We are seeking a detail-oriented and compassionate Patient Service Coordinator / Outpatient Coordinator to join our Community Health team in a remote capacity. This role focuses on coordinating care, scheduling appointments, managing referrals, and supporting patients with access to healthcare and community resources. The ideal candidate is passionate about public health, patient advocacy, and improving health outcomes in underserved communities.

🔹 Key Responsibilities:

• Coordinate outpatient appointments, referrals, and follow-up care for patients.

• Act as a primary contact for patients, providers, and community organizations.

• Support patients in navigating healthcare systems, insurance, and available resources.

• Maintain accurate records and ensure compliance with privacy and documentation standards.

• Assist with telehealth coordination and remote patient support services.

• Identify barriers to care and connect patients with community programs (food, housing, transportation, etc.).

• Collaborate with interdisciplinary teams to ensure patient-centered care.

• Monitor patient progress and satisfaction through follow-ups and outreach calls.

🎓 Qualifications:

• High school diploma or GED required; Associate’s or Bachelor’s degree in Health or Social Services preferred.

• Community Health Worker (CHW) certification or equivalent experience highly desirable.

• Experience in outpatient coordination, case management, or patient services preferred.

• Strong communication, organizational, and computer skills (EHR systems, scheduling software, etc.).

• Ability to work independently in a remote environment.

• Bilingual (English/Spanish) is a plus.
