Position: Hospital Case Management Manager
Department: Case Management / Care Coordination
Reports To: Director of Nursing / Chief Nursing Officer
Overview:
The Hospital Case Management Manager oversees and coordinates patient care management activities to ensure quality outcomes, effective discharge planning, and appropriate resource utilization.
This leadership role supports interdisciplinary collaboration between nursing, physicians, and social work teams to enhance patient flow and optimize hospital efficiency.
Manage daily operations of the case management department.
Supervise Case Managers, Social Workers, and Care Coordinators.
Develop and implement discharge planning processes for all patients.
Ensure compliance with hospital policies, insurance requirements, and regulatory standards.
Collaborate with physicians and healthcare teams to facilitate continuity of care.
Analyze patient care data and prepare utilization and outcome reports.
Educate staff on best practices in case management and transitions of care.
Serve as a liaison between hospital administration and clinical departments.
Registered Nurse (RN) license or Master’s in Social Work (MSW) required.
Bachelor’s or Master’s degree in Nursing, Healthcare Administration, or related field preferred.
3–5 years of case management experience in a hospital setting.
Strong knowledge of discharge planning, utilization review, and insurance coordination.
Excellent leadership, communication, and problem-solving skills.
