📝 Job Summary:

We are seeking a compassionate and experienced Home Health RN Case Manager to provide high-quality nursing care to patients in their homes.
In this role, you will perform assessments, develop personalized care plans, deliver skilled nursing services, and coordinate care with physicians and interdisciplinary teams.
This position is ideal for RNs who value independence, flexibility, and patient-centered care.

🩺 Key Responsibilities:

Conduct comprehensive patient assessments
Develop, implement, and update individualized care plans
Provide skilled nursing care including wound care, IV therapy, and medication management
Educate patients, families, and caregivers on health conditions and care techniques
Document all services and updates in the EMR system
Coordinate care with physicians, therapists, social workers, and other team members
Monitor changes in patient condition and adjust care plans as needed
Follow agency policies, clinical guidelines, and regulatory standards

🎓 Qualifications:

Active and unrestricted Registered Nurse (RN) License
Minimum 1 year of RN experience (Home Health experience preferred)
Valid driver’s license and reliable transportation
Experience with EMR documentation
Strong communication, clinical assessment, and critical thinking skills
