We are hiring a compassionate and skilled Registered Nurse (RN) specializing in Wound Care, Home Health Nursing, Geriatric Patient Care, and Clinical Case Management. The nurse will deliver high-quality evidence-based nursing care across multiple settings, ensuring patient safety, comfort, and recovery.
Perform thorough clinical assessments and create personalized nursing care plans
Provide advanced wound care management, including dressing changes, wound evaluation, infection control, and documentation
Deliver home health nursing services such as medication administration, IV therapy, vitals monitoring, and patient education
Manage elderly / geriatric patients with chronic illnesses focusing on mobility, emotional support, and long-term recovery
Coordinate with physicians, physical therapists, and interdisciplinary teams to optimize patient outcomes
Maintain accurate patient records using EMR / EHR systems
Educate patients & families on wound management, nutrition, safety, and home-care treatment compliance
Respond promptly to patient emergencies and follow escalation protocols
Valid Registered Nurse (RN License) — mandatory
1–3+ years of clinical nursing experience preferred
Experience in Wound Care / Geriatric / Home Health / Medical-Surgical / Clinical Care
Knowledge of infection control standards and assessment procedures
Strong critical thinking, decision-making, and communication skills
BLS / ACLS certification required (WOCN certification preferred)
