📌 Job Title: Licensed Practical Nurse (LPN) – Skilled Nursing Facility (SNF)

🩺 Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team in a Skilled Nursing Facility (SNF) setting. The ideal candidate will provide high-quality patient care, support daily clinical operations, and ensure resident well-being through skilled nursing practices.

🔹 Key Responsibilities

1. Resident Care & Clinical Support

Provide direct nursing care to residents in a Skilled Nursing Facility environment.
Monitor vital signs, administer medications, and document all treatments accurately.
Assist with wound care, IV therapy (as permitted), catheter care, and routine assessments.
Support residents with ADLs and mobility as needed.

2. Care Coordination

Collaborate with RNs, CNAs, Physicians, Therapists, and Interdisciplinary Teams to ensure optimum resident care.
Communicate changes in resident conditions promptly and clearly.
Participate in care planning and follow personalized care plans.

3. Documentation & Compliance

Maintain accurate and timely medical records according to facility policies and state regulations.
Ensure all charting meets SNF regulatory and compliance standards.
Adhere to infection prevention and medication administration protocols.

4. Patient Safety & Quality Care

Promote a safe and supportive environment for residents.
Respond quickly to medical emergencies and provide appropriate interventions.
Ensure dignity, comfort, and patient-centered care at all times.

5. Family & Resident Interaction

Provide emotional support and clear communication to residents and families.
Educate families on care plans, medication, and health updates.

🎯 Qualifications

Valid LPN/LVN License in state of practice.
Experience in Skilled Nursing Facility (SNF) preferred.
Strong clinical, assessment, and communication skills.
Ability to work effectively in a fast-paced, team-oriented environment.
Knowledge of EMR systems is a plus.
