Position Summary:
The Resident Care Coordinator is responsible for overseeing daily clinical operations, ensuring the highest standard of resident care, coordinating care plans, supervising nursing staff, and maintaining compliance with healthcare regulations. This role requires strong leadership, clinical expertise, and excellent communication skills.
Develop, update, and monitor individualized resident care plans.
Coordinate assessments, documentation, and resident evaluations.
Supervise and support nursing staff (RNs, LPNs, CNAs).
Communicate with physicians, families, and interdisciplinary teams.
Ensure adherence to clinical policies, safety protocols, and state regulations.
Monitor medication administration and treatment procedures.
Participate in quality assurance programs and audits.
Provide training, mentorship, and performance feedback to staff.
Respond to resident emergencies and clinical concerns promptly.
Maintain accurate records, charts, and care documentation.
Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN).
Active state nursing license in good standing.
1–3 years of clinical or long-term care experience preferred.
Strong leadership, organizational, and critical-thinking skills.
Excellent communication and resident-focused approach.
Knowledge of state and federal healthcare regulations.
Experience with EHR systems preferred.
