Position Summary:
The LTC Social Worker supports residents and families in long-term care settings by providing emotional support, coordinating care plans, assisting with transitions, and ensuring resident rights and well-being. This role requires strong communication skills, empathy, and knowledge of healthcare regulations.
Conduct psychosocial assessments for new and existing residents.
Develop and update individualized care plans in collaboration with the interdisciplinary team.
Provide counseling, emotional support, and crisis intervention to residents and families.
Coordinate discharge planning and smooth transitions (hospital, home, rehab).
Advocate for resident rights and ensure compliance with LTC regulations.
Manage admissions, transfers, and family meetings.
Assist residents with financial, social, and community resources.
Maintain complete and accurate documentation in EHR systems.
Support grief, loss, adjustment issues, and mental well-being.
Participate in quality assurance, audits, and regulatory surveys.
Bachelor’s or Master’s degree in Social Work (BSW/MSW).
Valid state Social Worker license (LSW/LMSW/LCSW).
Experience in long-term care, skilled nursing, or healthcare settings preferred.
Strong communication and case-management skills.
Knowledge of Medicare/Medicaid and LTC regulations.
Ability to work with elderly, disabled, and diverse populations.
