LTC Social Worker – Job Description

Position Summary:
The LTC Social Worker supports residents and families in long-term care settings by providing emotional support, coordinating care plans, assisting with transitions, and ensuring resident rights and well-being. This role requires strong communication skills, empathy, and knowledge of healthcare regulations.


Key Responsibilities

  • Conduct psychosocial assessments for new and existing residents.

  • Develop and update individualized care plans in collaboration with the interdisciplinary team.

  • Provide counseling, emotional support, and crisis intervention to residents and families.

  • Coordinate discharge planning and smooth transitions (hospital, home, rehab).

  • Advocate for resident rights and ensure compliance with LTC regulations.

  • Manage admissions, transfers, and family meetings.

  • Assist residents with financial, social, and community resources.

  • Maintain complete and accurate documentation in EHR systems.

  • Support grief, loss, adjustment issues, and mental well-being.

  • Participate in quality assurance, audits, and regulatory surveys.


Qualifications

  • Bachelor’s or Master’s degree in Social Work (BSW/MSW).

  • Valid state Social Worker license (LSW/LMSW/LCSW).

  • Experience in long-term care, skilled nursing, or healthcare settings preferred.

  • Strong communication and case-management skills.

  • Knowledge of Medicare/Medicaid and LTC regulations.

  • Ability to work with elderly, disabled, and diverse populations.
