The Hospice RN Case Manager provides compassionate, patient-centered care to individuals with terminal illnesses, focusing on comfort, dignity, and quality of life. This role coordinates and manages the overall hospice care plan, collaborates with interdisciplinary teams, supports patients and families emotionally and clinically, and ensures care aligns with hospice philosophy and regulatory standards.
Conduct comprehensive patient assessments and ongoing evaluations
Develop, implement, and update individualized hospice care plans
Manage pain and symptom control, including medication administration
Coordinate care with physicians, social workers, chaplains, and hospice team members
Provide education and emotional support to patients and families
Perform skilled nursing visits in home, facility, or inpatient hospice settings
Monitor changes in patient condition and adjust care plans accordingly
Ensure accurate and timely documentation in EMR systems
Participate in interdisciplinary team meetings and care conferences
Ensure compliance with hospice regulations, infection control, and quality standards
Active Registered Nurse (RN) license
Hospice or palliative care experience preferred
Strong assessment, care coordination, and case management skills
Expertise in pain management and end-of-life symptom control
Excellent communication, empathy, and emotional intelligence
Ability to work independently and manage a patient caseload
Knowledge of hospice regulations, Medicare guidelines, and EMR systems
Valid driver’s license for community-based visits
