Job Title: Social Worker Acute Care
Job Summary:
As a Social Worker in Acute Care, you will be responsible for assessing and addressing the psychosocial needs of patients in a hospital setting. You will work closely with medical staff to develop treatment plans and provide support to patients and their families during times of crisis.
Responsibilities and Duties:
1. Conduct psychosocial assessments of patients to determine their needs and develop individualized care plans.
2. Provide counseling and emotional support to patients and families facing medical crises.
3. Collaborate with medical staff to coordinate discharge planning and ensure a smooth transition to post-hospital care.
4. Advocate for patients and ensure their rights are protected throughout the healthcare process.
5. Connect patients and families with community resources and support services.
6. Document all interactions and interventions in patient records.
Qualifications and Skills:
1. Master's degree in Social Work from an accredited program.
2. Current licensure as a Licensed Clinical Social Worker (LCSW).
3. At least 2 years of experience in a healthcare setting, preferably in acute care.
4. Strong communication and interpersonal skills.
5. Ability to work effectively as part of a multidisciplinary team.
6. Knowledge of relevant laws and regulations pertaining to healthcare and social work.
