Job title: Long Term Care (LTC) LPN
Job Summary:
As a Long Term Care (LTC) LPN, you will be responsible for providing compassionate and high-quality nursing care to residents in long-term care facilities. You will work closely with other healthcare professionals to assess residents' needs, develop care plans, administer medications, and provide ongoing support and assistance to residents and their families.
Responsibilities and Duties:
- Assessing residents' health status and developing individualized care plans
- Administering medications and treatments as prescribed by healthcare providers
- Monitoring residents' vital signs and reporting any changes to the appropriate team members
- Providing emotional support and guidance to residents and their families
- Collaborating with other healthcare professionals to ensure coordinated and comprehensive care
- Documenting all care provided and maintaining accurate records
- Following all facility policies and procedures to ensure the safety and well-being of residents
Qualifications and Skills:
- Valid LPN license in the state of practice
- Experience working in long-term care or a similar healthcare setting
- Strong communication and interpersonal skills
- Compassionate and patient-focused approach to care
- Ability to work effectively in a fast-paced and demanding environment
- Knowledge of relevant healthcare regulations and best practices
