Job Title: Home Health/Community Health Manager
Job Summary:
We are seeking a Home Health/Community Health Manager to oversee and coordinate our home health and community health programs. The ideal candidate will be responsible for managing a team of healthcare professionals, ensuring quality patient care, and developing and implementing policies and procedures to improve the health outcomes of our patients.
Responsibilities and Duties:
- Manage a team of healthcare professionals, including nurses, therapists, and home health aides
- Coordinate and oversee patient care plans, ensuring they meet the individual needs of each patient
- Develop and implement policies and procedures to improve patient care and outcomes
- Oversee the scheduling of patient visits and ensure timely and efficient delivery of services
- Monitor and evaluate the performance of staff members, providing coaching and feedback as needed
- Collaborate with community organizations and healthcare providers to promote health and wellness in the community
- Ensure compliance with all state and federal regulations governing home health and community health programs
Qualifications and Skills:
- Bachelor's degree in healthcare administration, nursing, or related field
- Minimum of 3 years of experience in home health or community health management
- Strong leadership and communication skills
- Knowledge of state and federal regulations governing home health and community health programs
- Ability to multitask and prioritize in a fast-paced environment
- Strong analytical and problem-solving skills
