Job Title: Registered Nurse, MDS Coordinator, Long Term Care (LTC)
Job Summary:
We are seeking a dedicated and experienced Registered Nurse to join our team as an MDS Coordinator in a Long Term Care facility. The MDS Coordinator is responsible for coordinating and completing resident assessments to ensure compliance with federal and state regulations. The role involves collaborating with interdisciplinary teams to develop and implement care plans for residents.
Responsibilities and Duties:
1. Conduct resident assessments using the Minimum Data Set (MDS) tool.
2. Collaborate with healthcare providers to develop comprehensive care plans for residents.
3. Monitor and update resident care plans as needed.
4. Ensure compliance with federal and state regulations related to resident assessments.
5. Provide education and training to staff on MDS documentation and regulations.
6. Participate in quality improvement initiatives to enhance resident care.
7. Communicate with residents and family members regarding care plans and assessments.
8. Coordinate with external agencies and providers to support resident care needs.
9. Maintain accurate and up-to-date documentation of resident assessments and care plans.
10. Participate in interdisciplinary team meetings to discuss resident care and progress.
Qualifications and Skills:
1. Valid Registered Nurse license in the state of practice.
2. Minimum of 2 years of experience in long term care or similar setting.
3. Certification as a MDS Coordinator preferred.
4. Strong understanding of MDS process and regulations.
5. Excellent communication and interpersonal skills.
6. Ability to work collaboratively with interdisciplinary teams.
7. Attention to detail and accuracy in documentation.
8. Knowledge of long term care regulations and guidelines.
9. Ability to prioritize and manage multiple tasks effectively.
10. Proficiency in computerized documentation systems.
